Skin Rejuvenation Procedures—No down time

All of our skin rejuvenation procedures are either performed by Dr. Putman or a Licensed Esthetician. Other nonablative laser treatments and injections may be performed by a specially trained Registered Nurse. Procedures marked with a * are performed by our Estheticians who are under the direct supervision of Dr. Putman. There are risks associated with laser and light based procedures, but those risks are minimized greatly at the Facial Plastic & Laser Surgery Center due to the level of training, our expertise in the field, and our long-term commitment to safety. Our lasers and light based equipment are classified as medical devices and we believe that medical devices should ideally be used in an environment where there is direct physician supervision. Dr. Putman is available to address any concerns about the skin before or after a treatment.

Injectable Filling Agents:

Restylane, Perlane, Juvederm Ultra and Juvederm Ultra Plus – Several very popular forms of injectable stabilized hyaluronic acid gel have been FDA approved in the United States. These are very safe and effective for wrinkle correction, lip enhancement and facial contouring. Hyaluronic acid is a biodegradeable, but long-lasting injectable agent. It produces a very natural augmentation effect. This can be used for fine lines or for larger volume augmentation, such as nasolabial folds and for a very effective lip augmentation. The augmentation is usually performed using some adjunctive local anesthetic and can be expected to last for six months or more, depending on the product and areas treated.

Radiesse – Radiesse is an FDA approved biocompatible implant material for long lasting soft tissue augmentation. Radiesse is used in facial augmentation procedures including treatment of nasolabial and melolabial folds, correction of scars, lines and wrinkles and soft tissue augmentation, as well as usage to augment skeletal or bony features in some instances. Side effects are minimal, but can include temporary bruising. Correction obtained with this agent is long lasting, but usually a second injection is required at about 4-6 months. This can produce a significant correction last 12-24 months, depending on the areas treated. Radiesse is not used for lip augmentation, but is a very effective volume filler for deep folds and wrinkles. Injections are performed using adjunctive local anesthetic.

Other fillers are now on the market, but Dr. Putman prefers to use those detailed above because of their combination of safety, efficacy and longevity.

Botox

Botox Cosmetic – Botox (botulinum toxin Type A) is a purified neurotoxin complex which acts as a nerve impulse "blocker." When injected in small concentrations and volumes into hyperdynamic facial muscles, it causes a temporary weakness or paresis of the muscle, as well as progressive atrophy. Because of this, Botox was initially used for treatment of neuromuscular twitching disorders. However, patients that were injected with this agent also noticed a significantly refreshed and happier look because of weakening of the frown muscles, and at times some medial eyebrow elevation. Consequently, Botox has been used to treat hyperdynamic muscle creases and frown lines in the following areas:

  • glabella ( frown lines)
  • forehead
  • crows feet
  • "Bunny" lines (nose)
  • lip lines ("smokers lines")
  • drooping corners of mouth
  • neck lines

The effects of Botox last approximately 4 months, but this can vary from 3-6 months. Repeated injections may result in more prolonged benefits. The clinical effect is often quite dramatic and can also be improved with injectable collagen fillers or laser resurfacing. The technique has been performed on thousands of patients with the most frequent side effects being short lived transient headache or irritation at the injection site. Botox cannot replace the dramatic effects that can be obtained with certain endoscopic procedures, but it is a very effective treatment for localized facial problems, as noted above. A brief consultation is required for this procedure.

Iriderm 532 Diode Laser:

The 532 diode laser is a very affective nonablative treatment for vascular and pigmented lesions. These include telangiectasia, spider veins, red hemangomas, sun spots, and lentigines. Only topical anesthesia using cold gel packs or ice is necessary to perform the procedure. A computer pattern generator is used to treat larger areas uniformly. Protective eyewear is worn throughout the procedure. Healing is very rapid, and the only side affects include temporary redness for a few hours.

*Comprehensive Skin Care Program:

We also provide comprehensive, individualized skin care programs for our patients. A good skin care program is fundamentally important for vibrant, healthy skin and complexion. It is also important for the prevention of degenerative disorders and skin cancer.

Our skin care program utilizes facial and body products, cleansers, moisturizers, pigment control agents and specialty products, including AHA's, retinoids, acne control agents, organic vitamins A, C, antioxidant products, and retexturing programs utilizing periodic PCA peels and microdermabrasion using the Diamondtome microdermabrasion system. For pigmentation problems, telangiectasia, sun damage, or other skin problems, light based treatments such as IPL photofacials, nonablative laser and LED are quite beneficial. We also recommend appropriate oral vitamins, purified fish oil supplements and an antiaging diet.

Our staff and I will help you formulate an individualized skin care program based on your particular skin care problems or needs. You need not substitute for products that you currently use and are pleased with.

*Intense Pulsed Light (IPL):

Photo Facials – IPL treatments use light energy for photorejuvenation. As the skin ages and is exposed to the elements of nature, it suffers from degradation and imperfections. Using IPL, short pulses of light selectively target and coagulate chromophores in the epidermis and papillary dermis. Oxy-hemoglobin in vascular lesions and melanin in pigmented lesions are coagulated. The coagulated cells are eliminated by a natural process and replaced by new cells. For fine lines and wrinkles, IPL selectively inflicts micro thermal trauma in the dermal-epidermal junction. The trauma triggers a wound-healing process and new collagen remodels under an undamaged outer layer. IPL treatments result in clearer, younger looking skin.

Photodynamic Intense Pulsed Light Therapy :

By adding a photosensitizing agent before a photo facial or acne treatment, great results can be achieved in just 1-2 treatments. A solution of 5-aminolevulinic acid (ALA) is applied to the skin 1-2 hours prior to treatment. ALA is a substance that occurs naturally in the body. When applied topically, it is selectively absorbed into fast-growing, rapidly metabolizing epidermal cells, including photodamaged skin, sebaceous glands, hair follicles and actinic keratoses. ALA is converted to a powerful photosensitizer. Followed by this, the activation by IPL produces the singlet oxygen that destroys the target cell.

After Photodynamic Intense Pulsed Light Therapy, it is important that the area treated not be exposed to the sun for 48 hours. There can be a strong degree of redness and skin may slough or peel for 1-3 days.

Light Emitting Diode Therapy:

Light emitting diode therapy is a new form of non-laser phototherapy, which has many beneficial medical uses. These include acceleration of wound healing, certain types of cancer therapy, and many dermalogic applications including treatment of psoriasis, acne and photo rejuvenation of sun damaged skin. The Facial Plastic and Laser Surgery Center offers LED light treatments as part of our comprehensive skin care program. This therapy helps reverse sun damage and skin degeneration by stimulating the increased production of collagen by the fibroblasts in the skin. It also decreases redness, fine lines and wrinkles, brown spots, and pore size in the skin over time. It is particularly beneficial in reducing redness and erythema after laser resurfacing, chemical peels, or a bad sunburn. LED therapy is another nonablative, no-downtime treatment modality, which is an integral part of an ongoing skin care rejuvenation program. It is used in combination with other modalities such as IPL photo facials, microdermabrasion, and Accent radiofrequency skin tightening, as well as Physicians Choice skincare products.

LightSheer Diode Laser for Permanent Hair Reduction and Treatment of Spider Veins:

Our office is pleased to offer state of the art technology in reduction of unwanted hair and treatment of small leg veins or telangiectasias using the LightSheer diode laser system. This system is clearly considered the gold standard in treatment of these problems and offers a patented Chill Tip handpiece which actively cools and protects the skin during the treatment procedure. A series of treatments is necessary to obtain effective long-term and permanent hair reduction because only follicles in the active growth phase will respond to the treatment. However, as few as four to six treatments may be effective in obtaining up to 87% permanent hair reduction in a given area according to documented research with this system. No other laser system or IPL treatment has proven to be more effective.

The LightSheer can also be used to treat small bluish and purple leg veins, which persist following sclerotherapy, which is still the recommended primary treatment for this condition.

A New Wrinkle In Skin Tightening – Accent:

Accent is a new skin tightening procedure for the nonablative and noninvasive treatment of skin laxity and wrinkles. This new technology utilizes a unique temperature-controlled delivery system of radiofrequency waves to the dermis to effect skin tightening and subsequent collagen remodeling, as well as stimulation of new collagen formation over a period of three to six months. While this procedure does not accomplish the dramatic skin tightening effects of laser resurfacing or surgical procedures, it is beneficial and effective for patients with mild to moderate skin laxity and wrinkles even if they have had previous surgery or laser resurfacing. It is also appropriate for patients who have had mild brow ptosis or facial or neck laxity who are not yet candidates for surgical rejuvenation and wish to avoid any downtime or time off of work. Accent does not eliminate muscle laxity or fatty deposits, but it can be used effectively in conjunction with our other skin care programs including microdermabrasion, peels, IPL “photo facials,” Botox Cosmetic, other injectable agents listed on this web site, and nutritional programs to optimize skin care and maintenance of a youthful, rested appearance.
